Medical Forms(click drop down option to view items)

All medical forms may be accessed on the Back to School Resources page on the Regina website. The Illinois Department of Public Health (IDPH) mandates a physical for entry into 9th grade. Transfer students must have completed medical forms on file. As IDPH mandates, seniors must submit a current immunization record with proof of Meningitis vaccination on or after 16 years of age. If your daughter will require any over-the-counter medication, a Medication Authorization form must be completed each school year. Regina Dominican Morning Bus Program
 

Morning bus service for freshmen begins on Thursday, August 15th and for all other students on Monday, August 19th. More information on pick up times will be available once the routes are determined and your student’s bus registration is complete. Please read ALL of the information below for detailed information on how to register. 

    • Routes and pick up locations are determined based on interest.
    • Registration will be on a first come, first serve basis. Once a route is full, we will not be able to accommodate additional riders.
    • Priority for ridership will be given to returning riders first and then to freshmen and sophomores. This priority registration opens on Wednesday, July 3rd at 9:00 am and closes on Monday, July 15th at 9:00 am. Please click here to access priority bus registration for 9th and 10th graders.
    • To secure your student’s seat on the bus, you must complete both the registration form and the payment.
    • Payment for the morning bus service will be $500 per semester/$1000 per year.  The second semester payment will be charged to your FACTS account in December. To secure your student’s seat, you must submit the first semester payment at the time of registration.

Student Parking                                                      

To register for a student parking permit, please read the following information:

  • Parking is by permit only and must be updated each school year.
  • Student parking permits are distributed on a first come, first serve basis and must be hung from the rearview mirror while the car is parked on property.
  • Parking permits will be $275 annually. 
  • Students with at least one other passenger will be offered a discounted carpool rate of $225 annually.
  • Parking permits will be distributed on the first day of school.
  • To secure a parking permit, you must complete both the registration form and payment will be billed to your FACTS account. The registration opens on Wednesday, July 3rd at 9:00 am.  Please click here to register for a student parking permit.

  • All students are required to have a laptop for use in their classes. No student will be permitted to use an iPad for their classes. While no specific brand of laptop is recommended, Google Classroom and Microsoft Office Software should be accessible. With classrooms becoming increasingly paperless, a tablet-style laptop is recommended to allow students to take notes and annotate digitally. PLEASE NOTE:  We do not recommend Chromebooks because they are not supported by the College Board, which administers AP curriculum as well as PSAT, SAT, and AP tests to our students. Regina does not offer loaner laptops for AP or standardized tests.
  • CALCULATOR: All students are required to have a graphing calculator. Recommended brand is the TI 84 Plus Family.
